$100 flat fee paid upon scheduling. Medications will be sent to your local pharmacy if appropriate.
Initial Consult (New patient) - $100 for 30 min session plus cost of medication options that suit your individual needs
Follow up - $100 per 30 min session (check in to assess medication and review results)
Urgent Care Visit - $100 per 20 min consultation
Essential Membership - $149/mo (6 month minimum). Includes one 30 minute virtual visit every 3 months, medication management (including retail pharmacy medications when appropriate), and secure messaging with 48-72 hour response time - Schedule an initial consultation to discuss this option and learn more
Enhanced Membership - $249/mo (6 month minimum). Includes one 30-45 minute virtual visit every month, medication management (including retail pharmacy medications when appropriate), and secure messaging with 24-48 hour response time - Schedule an initial consultation to discuss this option and learn more
***Once you schedule a time that will work for you, you will be emailed instructions for the portal. I do recommend you sign up for the portal, as you will be able to access lab and imaging results here. Additionally, please fill out all consent forms that are sent to you in that initial email prior to our first visit. This is a requirement to be seen. Payment is due at the time of booking.
I do not bill insurance but am happy to supply you with an invoice so that you can submit to your insurance. This is also known as a "Superbill".
